What does an assessment involve?

A Home Support Assessment is a face-to-face holistic assessment of a person’s needs.  The Assessor will work with individuals, carers and families to develop a support plan to reflect the individual’s needs, goals and preferences.

What happens after the assessment has been completed?

If you are eligible for aged and disability services the Regional Assessment Service will link you into a range of supported services such as

  • Home Support Services (Domestic Assistance, Personal Care, Respite, Social Support)
  • Allied Health (Occupational Therapy, Dietician, Social Work, Physiotherapy, Podiatry)
  • District Nursing
  • Planned Activity Groups
  • Multicultural Services
  • Aged Care Assessment Team (Residential Care and Aged Care Packages)
  • Disability Services
