Home and Community Care Program for Younger People
The Home and Community Care Program for Younger People (HACC PYP) supports Victorians from birth to 65 years, and Aboriginal and Torres Strait Islander people from birth to 50 years if their capacity for independent living is at risk. This program supports eligible individuals who require one-off, intermittent, or ongoing support to undertake the activities of daily living.
Within South Gippsland, services are delivered by various service provider organisations, and not directly by South Gippsland Shire Council.
To access HACC PYP in South Gippsland, the first step is to complete an eligibility assessment, provided by Mecwacare, who can be contacted via email intake@mecwacare.org.au or phone (03) 8573 4980.
