Emergent Work Details	What is the damage, e.g. ‘3x large trees and multiple branches on asset’ or ‘hangers and fallen tree limbs impacting road’
Work Team	Team or Contractors assigned to rectify
Completed	Only tick yes when all works are completed
Primary Location and Location	Should automatically appear but can be searched if required
Chainage From	Should Auto populate
Chainage To	Please refresh at other end of damaged area
Lat/Lon	Should Auto Populate from submission location
Lat/Lon To	Please refresh at other end of damaged area
Damage Type	What damage is present e.g. ‘Unsealed road surface damage’
Response Type	Immediate Reconstruction within 3 months of damage or REPA works with Certified Estimate
Damage Details	Detail of damage and if possible reason for damage e.g. ‘Severe Potholes to 300m of road due to flooding’
Treatment Type	Standard Treatments as provided in the DRFA
